CDL Medical Card

As of May 2014, CDL holders are required to obtain a Federal Motor Carrier Safety Administration (FMCSA) medical examination from certified personnel, like Dr. Marrone. Under the FMCSA National Registry, CDL holders can rest assured that they are meeting with a qualified professional who will abide by Federal regulations and guidelines.

Certified medical examiners listed on the FMCSA registry can examine commercial drivers from any state. In order to maintain a CDL Class A/B/C or CDL permit, drivers must receive a DOT medical examiner’s certificate. Forms are available at Northeast Ohio DOT Physicals.

Before Your Appointment

What to bring to your CDL Medical Exam:

  1. Driver’s License
  2. Current CDL Medical Card (if applicable)
  3. Eyeglasses, Contacts, or Hearing Aids (if applicable)
  4. A list of medications with dosage

Those with the following diagnoses will need to follow these further guidelines:

  • Diabetes
    • Bring your most recent A1C reading from your physician
  • Cardiac condition including history of heat attack or previous heart surgery
    • Requires a letter of clearance to safely operate a commercial vehicle from your cardiologist, including a normal EKG
  • Mental Health Condition
    • Letter of Clearance from a mental health professional indication your condition is stable and any medications do not interfere with your ability to safely operate a commercial vehicle
  • Sleep Apnea
    • You must use a CPAP mask and present a printed compliance of 70% or greater

A CDL Medical Card issued after passing the DOT physical is valid for 24 months. In some cases, such as monitoring a condition (i.e. high blood pressure), it may be necessary to issue a CDL Medical Card that is valid for less than 24 months.
